Duties Description The Office for People with Developmental Disabilities (OPWDD) provides supports and services to approximately 126,000 people with developmental disabilities. OPWDD's vision is for people with developmental disabilities to enjoy meaningful relationships with friends, family and others in their lives; experience personal health and growth; live in the home of their choice and fully participate in their community.
Duties and responsibilities related to this position include, but are not limited to, the following:

· The POS 2 is part of OPWDD's Community Transitions Unit (CTU), which supports the Developmental Disabilities Regional Field Offices (DDRFOs) in ensuring people transitioning from institutional settings such as residential schools, developmental centers, hospitals, and nursing homes to less restrictive community placements have the support and planning needed for successful transitions. OPWDD's CTU works closely with DDRFO staff to ensure the services and supports necessary for these transitions are identified and secured.

· The POS 2 has the critical role of ensuring people with developmental disabilities who are preparing to leave residential or local schools are transitioned to the most appropriate adult services in a timely manner. The POS 2 works to facilitate the availability of adult services for students as close as possible to the time of their graduation dates, to prevent or limit the time they remain in school settings which are inappropriate to their service needs and which result in significantly higher costs to NYS. Reducing the number of people in residential schools who remain past their graduation dates is a high priority for OPWDD and New York State.

· The POS 2 supports the Local School Transition Coordinators and assists with the development of public facing Local School Transition materials. The POS 2 collects data on children's use of OPWDD services. The POS 2 leads regular statewide meetings with LSTCs to develop best practices and identify concerns and issues that require elevation.

· The POS 2 serves as a liaison between OPWDD and other New York State agencies when cases involve other service systems. This includes, but is not limited to, the New York State Education Department (NYSED), Office for Mental Health (OMH), Office of Children and Family Services (OCFS), New York City Administration for Children's Services (ACS), and local Departments of Social Services (DSS) on a statewide basis. The POS 2 assists in ensuring there is both open communication with these agencies and timely and appropriate placement for people who transition into OPWDD services. In addition, the POS 2 works with the OPWDD's regional statewide Residential School Transition Supervisor providing technical assistance and coordinating and standardizing processes and policies across the state.

· The POS 2 assists in oversight of the Residential School Transitions database, the development of required reports and ensures that all data requests are answered promptly. The POS 2 ensures access to the Residential School Transition (RST) and Local School Transition (LST) SharePoint sites and that they are kept current.

· This position requires a person with demonstrated ability to plan and execute strategic decisions, implement critical management and business practices, and deliver the outcomes expected by the individuals we serve.

Minimum Qualifications Promotion:
Candidates must be reachable for appointment on the current Civil Service Eligible List.

Transfer:
Candidates must have one year of permanent competitive or 55-b/c status in the posted title and be eligible for lateral transfer; OR be eligible to transfer via Section 70.1, 70.4, or 52.6 of Civil Service Law. The transfer cannot be a second consecutive transfer resulting in an advancement of more than two salary grades.

Additional Comments • If you are currently a New York State employee serving in a permanent competitive qualifying title and eligible for a transfer via Section 70.1 or 52.6 of the Civil Service Law, you may be appointed to this title as a permanent competitive employee.
