Duties Description Oversees and manages Intermediate Care Facility (ICF) operations.
• Develops and implements ICF policy and residential procedures.
• Establishes performance standards and observes treatment activities to determine program effectiveness.
• Reviews residential programs for compliance with federal, State, and certification requirements, regulations, and guidelines; modifies programs based on individuals needs and abilities.
• Participates in the development and implementation of individuals service plans; communicates program goals, objectives, and individuals progress and/or changes in condition to parents and/or family members.
• Monitors individuals cases, progress, and reports to verify that service plans and assessments are completed and individuals needs are being met appropriately.
• Oversees records maintenance activities to ensure compliance with certification, regulatory, and legal requirements.
• Works with the Business Office to monitor and control the use of ICF funds according to approved spending plans; approves requisitions for supplies, materials, and equipment; and ensures appropriate documentation of financial activities.
• Oversees physical ICF operations, including maintenance and housekeeping to ensure residents health and safety.
• Coordinates inspections performed by outside agencies, completes Plans of Corrective Action in response to Statements of Deficiencies when necessary, and follows up on citations.
• Responds to crisis and emergency situations as needed during both work and non-work hours.
• Initiates, conducts, and reports investigations of possible neglect and abuse at the ICF; analyzes findings, and makes recommendations.
Supervises, coordinates, and directs the activities of clinical and support staff.
• Oversees staff assignments and reassignments, evaluates performance, monitors time and attendance, and approves the use of overtime.
• Communicates and provides guidance to staff regarding program objectives, treatment techniques, and ICF policy and procedures.
• Participates in employment activities; identifies training needs and arranges for the provision of training.
Serves as liaison with the community.
• Develops and maintains positive community relationships with neighbors, businesses, and other service providers to improve ICF services.
• Represents the ICF in conferences, meetings, and public gatherings to communicate program and administrative information, discuss issues related to ICF operations, and encourage volunteer participation in programs.

Attend all pre-admission meeting for ETU, CATH and SU to provides rehabilitative incentives. Conduct reviews of habilitation evaluation to ensure routine submissions are met.
• Monitor individuals progress to verify comprehensive service plans are completed and individual needs are being met appropriately.
• Conduct reviews of minor and reportable incidents. Initiate minor investigations to ensure minor incident summaries are written and prepared for presentation at weekly FMP meetings. Ensure all requested documents and findings are gathered and accompany the appropriate incident report. Schedule and facilitate monthly incident trends meeting and complete appropriate grids to reflect incident management.

Description of Locations-
CATH 1
The New York State Office for People with Developmental Disabilities (OPWDD) Comprehensive Adult Transitional Home (CATH) program model provides intensive residential and day habilitation services for individuals with intellectual and developmental disabilities (IDD) discharging from campus-based and institutionalized settings. The CATHs are designed primarily to serve young adults transitioning from residential schools into OPWDD adult residential programs, however, may also serve individuals with high acuity needs, whose clinical presentation would be best met by the comprehensive treatment provided by the CATH. The CATH is designed to provide acute, time-limited treatment to help the individual meet the behavioral and/or psychiatric stabilization needed for placement in community-based setting, which includes fading of the intensive supports and restrictive elements employed in the previous setting. The current CATH model provides targeted intervention and person-centered treatment for individuals who have completed their educational goals and aged out of their NYS Education Department (NYSED) program.
ETU
The New York State Office for People With Developmental Disabilities (OPWDD), in collaboration with the Office of Mental Health (OMH) and Kings County Hospital (KCH) in Brooklyn, NY, operate two specialized programs designed specifically to provide intervention and person-centered treatment for individuals who are dually diagnosed with intellectual/developmental disabilities and mental illness/behavioral health needs, and who in are in an active period of psychiatric/behavioral health crisis. These specialized programs create a service pathway accessible to dually diagnosed individuals in periods of crisis. This pathway begins with a clearly established referral, screening and intake process. Individuals who meet eligibility criteria will receive admission to the Kings County Hospital (KCH) psychiatric inpatient unit where they will participate in approximately six-weeks of crisis intervention and intensive treatment.
In cases where additional active treatment is warranted, OPWDD will provide further specialized psychiatric and behavioral habilitation services for up to six-months through the Extended Treatment Unit (ETU). The OPWDD ETU provides specialized psychiatric and behavioral habilitative services for up to twelve individuals at any given time. Services at the ETU are person-centered and maintain a focus on continuous assessment, active treatment and discharge planning to assure a successful return to the individual’s home or community-based setting following treatment. The ETU will operate with a high staff to person ratio. ETU staff are provided with specialized training alongside professional clinicians dedicated to the treatment milieu.
Sensory Unit
The Sensory Unit is designed to provide residential opportunities for individuals currently residing within Bernard Fineson State Operations specifically from either ICF specialized unit where it is determined if the individual will benefit from or has limited responses for a potential placement. Referrals will be established through recommendations from the clinical team and approved by Administration as circumstances dictate. The selected individual will identify as having sensory needs from the 8 sensory systems or other specialty admissions authorized by OPWDD Bernard Fineson Leadership.

Minimum Qualifications Competitive:
You must be reachable for appointment on the current Civil Service eligible list used for filling this title.
Promotion:
One year as an Intermediate Care Facility Program Manager 1; or bachelor’s degree or higher in a human services field and one year of permanent service in a clinical or direct care position allocated to Grade 17 or higher.
Transfer:
Candidates must have one year of permanent competitive or 55-b/c status in the posted title and be eligible for lateral transfer; OR be eligible to transfer via Section 70.1, 70.4, or 52.6 of Civil Service Law. The transfer cannot be a second consecutive transfer resulting in an advancement of more than two salary grades.

Additional Comments • Please note that starting salary for this title reflects the downstate adjustment. Posted Hiring Salary includes the sum of the statutory hiring rate for PS&T Grade 19 ($70,579) and Downstate Adjustment ($4,000). Downstate Adjustment is also added to the job rate ($89,645).
