Duties Description • Will oversee, coordinate and assist in the development and provision of Residential services for individuals in an IRA setting.
• Will oversee and assist Hab Spec 1’s with the development and modification of Staff Action Plans, Safeguard Summary Plans, Money Management Assessments and Personal Expenditure Plans as needed.
• Will coordinate with Clinical staff to ensure all individual needs are met and programs are developed/modified as appropriate.
• Will provide training and hands on assistance to Hab Spec 1’s and staff in the proper delivery of daily services.
• Will provide training and oversight in the proper documentation of Medicaid Billing documents, Residential Habilitation Monthly Notes and General notes in relation to the implementation of residential services.
• Will develop and maintain a good working relationship with Care Managers and Day Habilitation staff that serve individuals.
• Will attend Life Plan reviews for individuals as needed, work collaboratively with the Care Managers and assist in the development and updating of Staff Action Plans and Safeguard Summary Plans that support the Life Plan.
• Will communicate with, develop and maintain as good working relationship with IRA managers and house staff to ensure the implementation of Staff Action Plans and staff resources are available for the needs of the individuals.
• Will supervise Hab Spec 1’s and carry a caseload of up to 13 Individuals.
• Will have oversight and be responsible for the content and organization of the individual program charts, house filing system and EHR in relation to Habilitation services.
• Will assist Hab Spec 1’s to provide oversight for the direct care staff in the development and implementation of Staff Action Plans and corresponding documentation.
• Will complete weekly and monthly chart audits to ensure documentation requirements are correct and present.
• Will communicate with Regional Office, CCO’s and Care Managers to ensure all current changes are brought to the Hab Spec for implementation.
• Will attend and participate in all mandatory trainings.
• Will run monthly Res Hab meetings and may be asked to chair committees for special programming.
• Will participate in quality review tours on a rotational basis.
• Other duties as assigned.
SELECTION CRITERIA:
• Ability to work as a team player.
• Ability to flex schedule to meet the needs of the individuals.
• Ability to manage time to meet the needs of the individuals, as well as documentation requirements.
• Previous documented and verifiable experience.
• Satisfactory employment history as evidenced by a review of personal history folder and supervisory evaluations.

Minimum Qualifications Promotion:
One year of service as a Habilitation Specialist 1 and reachable for appointment on the current Civil Service Eligible List for this title.

70.1 Transfer Qualifications:
Be eligible to transfer via Section 70.1 of Civil Service Law. Candidates must have one year of permanent competitive or 55b/c service in a title SG-16 or above deemed eligible for transfer under Section 70.1 of the Civil Service Law.

Additional Comments • If you are currently a New York State employee serving in a permanent competitive qualifying title and eligible for a transfer via Section 70.1 or 52.6 of the Civil Service Law, you may be appointed to this title as a permanent competitive employee.

• The assignment of PS&T employees will be made in accordance with any agreed upon Memorandum of Understanding between PEF and the local DDSO regarding the posting of job vacancies.
