Duties Description Responsibilities include strategic and operational leadership for all case management and discharge planning activities within the rehab facility. The incumbent will be Recruiting, training, and supervising case management staff to foster a collaborative team environment; coordinating care and communication among medical, nursing, therapy, and social services teams to facilitate safe, timely discharges; managing resources and caseloads to maintain service quality and efficiency; monitoring performance metrics and leading quality improvement efforts; acting as a liaison with patients, families, and post-discharge providers to ensure continuity of care; and contributing to budgeting, planning, and cross-functional initiatives as a member of the leadership team. They will also collaborate with external agencies and payers to support authorization and reimbursement processes. Will develop and implement policies and procedures to enhance case management efficiency and patient outcomes; and other duties as assigned.
Minimum Qualifications Transfers: Current New York State employee with one or more years of permanent, contingent-permanent, or 55 b/c service that is eligible for transfer under Section 70.1 and or 52.6 of the Civil Service Law to Social Work Supervisor 1, SG-23.
Promotion Provisional Appointment: In the absence of an eligible list with three or more candidates willing to accept a position, a temporary provisional appointment may be considered. For provisional appointment, candidates must meet be a current NYS Department of Health employee with one year of permanent competitive or 55-b/55-c service as a Licensed Master Social Worker, SG-20, and must have a license and current registration as a Licensed Master Social Worker in New York State.
Preferred Qualifications: • 3–5 years of experience in inpatient rehabilitation, skilled nursing, or post-acute care.
• Familiarity with CMS regulations, Joint Commission standards, and state-specific requirements related to rehabilitation care.
• Demonstrated success leading teams that include therapists, nurses, physicians, and social workers in a rehab environment.
• Ability to use data and outcome metrics to guide program improvements and demonstrate value-based care outcomes.
• CCM (Certified Case Manager), ACM (Accredited Case Manager), or similar certification

If there are three or fewer qualified candidates interested in the appointment to this position, a permanent appointment may be made pursuant to Section 52.7 of the Civil Service Law. To be eligible, candidates must be an employee of the Department of Health and have one year of permanent competitive service as a Licensed Master Social Worker, SG-20, and must be licensed and currently registered as a Certified Licensed Master Social Worker in New York State.
The posted salary range ($90,681 - $113,650) includes $4000 of downstate adjustment.

For new State employees appointed to graded positions, the annual salary is the hiring rate (beginning of the Salary Range) of the position. Promotion salaries are calculated by the NYS Office of the State Comptroller in accordance with NYS Civil Service Law, OSC Payroll rules and regulations and negotiated union contracts.
