Minimum Qualifications Must have a license and current registration to practice pharmacy in New York State.

Duties Description Provision of Pharmaceutical care, including the review, monitoring, dispensing and distribution of inpatient/outpatient meds. Responsible for providing drug info, conducting med counseling sessions, supervising Pharmacy Aides participating in Pharmacy and Facility wide performance improvement activities. You may be required to provide clinical and distributive services including Pharmacotherapy consults, Anticoagulation MGMT, Health and Wellness Program participation, dispensing and other clinical distributive services assigned. Actively manage drug therapy by identifying, preventing, resolving medication-related problems either directly or as part of the treatment team. Collect, analyze and report on med therapy related data to enhance patient safety and improve patient care and be responsible for maintaining safe, effective, and efficient operations in the pharmacy department involved in maintaining accurate dispensing, distribution, relate inventory records.

Additional Comments This is a Per-Diem position. Hours/work week will be discussed at the time of interview.
