Duties Description The Office for People With Developmental Disabilities (OPWDD) is seeking a pharmacist to complete Medication Regimen Review Services to maintain compliance with regulation 14 CRR-NY 633.17. Medication Regimen Reviews are required for those residing in certified Individual Residential Alternatives (IRA) and Intermediate Care Facilities (ICF) owned and operated by OPWDD throughout New York State. Medication Regimen Reviews are required for approximately 7,057 Individuals living in IRAs and approximately 445 Individuals living in ICF residences. Regulations require that Medication Regimen Reviews be conducted in ICFs on no less than a quarterly basis and in IRAs no less than an annual basis. Increased need for reviews may be required and requested in both IRA and ICF settings based on each Individuals medical conditions and necessities. Under the supervision of the Director of Pharmacy Services, this Pharmacist will perform statewide Medication Regimen Reviews in accordance with the scope of practice for pharmacy as defined in Article 137, Section 6801 of the New York State Education Law.

The primary duties for this position include, but are not limited to the following:

- Working within the framework of a treatment team providing services for Individuals diagnosed with developmental disabilities and their
caretakers/families.

- Reviewing and monitoring statewide medication regimens.

- Ensuring that a review of a Individual's medication regimen is conducted on no less than an annual basis in Individualized Residential
Alternatives (IRAs), and a quarterly basis in Intermediate Care Facilities (ICFs), or more frequently as needed.

- The medication regimen review shall include, at a minimum:

A review for potential adverse reactions, allergies, interactions, contradictions, or irregularities; related laboratory work shall
be included in this review.

An assessment of the Individual's response to medication therapy to determine if the medication is achieving the stated
objectives established by the prescribing practitioner.

Recommendations to the primary and/or consulting practitioner of any indicated changes in the Individual's medication regimen.

Determination of the need for a more frequent review depending upon the Individual's medical status.

Documentation of the review, findings, and any recommendations made.


- Providing technical assistance and guidance to DDSOO staff and/or community agencies about pharmacy medication policy and review standards.

- Other duties or tasks as assigned.

Minimum Qualifications Open Competitive: possession of a license and current registration to practice pharmacy in New York State.

Additional Comments This title is part of the New York Hiring for Emergency Limited Placement Statewide Program (NY HELPS). HELPS Program titles may be filled via a non-competitive appointment. This means that you do not need to take an exam to qualify, but you do need to meet the minimum qualifications of the title.

At a future date (within one year of permanent appointment), employees hired under NY HELPS are expected to have their permanent non-competitive employment status converted to permanent competitive status. You will not have to take an exam to gain permanent competitive status.

• Please note that starting salary for this title reflects the geographic differential and downstate adjustment. Posted Hiring Salary includes the sum of the statutory hiring rate for Grade 25 ($96,336), Geographic Differential ($11,000) and Downstate Adjustment ($4,000). Geographic Differential and Downstate Adjustment is added to the job rate ($121,413).
