Minimum Qualifications MINIMUM QUALIFICATIONS:

Licensed Master Social Worker 2 – Licensed and currently registered by New York State as a Licensed Master Social Worker (LMSW) and have one year of post NYS licensure professional social work experience; **

OR

Licensed and currently registered by New York State as a Licensed Clinical Social Worker (LCSW). Registration and licensure as a Licensed Mental Health Counselor are not considered qualifying.

This position will serve as a Licensed Master Social Worker for a division within the Developmental Disabilities Regional Office 2. Divisions within the Regional Office include the following: Service Access (Front Door and Waiver Services), Eligibility, Self-Direction, Housing, Crisis, Property/Development, Fiscal, and Quality Management/Technical Assistance.
Regional Office 2 supports over 1,800 individuals with developmental disabilities who reside in 20 counties of NYS: Broome, Cayuga, Chenango, Clinton, Cortland, Delaware, Essex, Franklin, Hamilton, Herkimer, Jefferson, Lewis, Madison, Oneida, Onondaga, Oswego, Otsego, St. Lawrence, Tioga, and Tompkins.

Supports are provided via a network of approximately 110 agencies who are the direct providers of services. Travel to the three districts within Region 2 may be required, as well as intermittent travel to other areas of the state.


PRIMARY DUTIES AND RESPONSIBILITIES:

• Collaborate with behavioral health & crisis services to coordinate of care for individuals transitioning out of highly specialized programs into OPWDD services.

• Develop local and statewide resource directories and technical guides.

• Serve as the liaison between providers serving people with complex and challenging support needs, responsible for coordinating needed services for these individuals and/or their families.

• Assist in the design and implementation of community-based treatment supports/programs focusing on relapse prevention, impulse and anger management, harm reduction and skill acquisition for increased self-management.

• Support individual(s) in crisis and the agency/program(s) supporting them, or to prevent imminent crisis and the agency/program(s) supporting them, or to prevent imminent crisis by provision of direct case consultations, professional mentorship, supervision, training, and program monitoring.

• Perform various assessments (risk, need, adaptive and functional) and review prior evaluations and clinical plans of care.

• Assist in the completion of consultation reports to include formal recommendations, referrals, treatment alternatives and service outcomes.

• Conduct screenings to assist in the evaluation of service needs.

• Perform clinical program, service access and quality reviews to ensure appropriateness of treatment/service provision.

• Member of Community of Informed Practice at the state and local level-to foster collaboration and build bridges to community-based services and participate in other committees/agency workgroups as delegated.
