Job Specifics
Duties Description The Medical Specialist 2 is a licensed, Board-Certified physician solely qualified to practice medicine in the state of New York. The Taconic district spans five counties including Putnam, Dutchess, Ulster, Columbia and Greene counties, and provides services to over 300 individuals with ID/DD, many of whom are advanced in age and have complex medical issues and physical challenges. The Medical Specialist 2 physician will serve approximately 50% of the time within the Article 16 Clinic, as the Article 16 Clinic Medical Director, and the other 50% of the time, as the State Operations Residential physician. Medical Specialist 2 hired for this position would either be a Board-Certified Neurologist or a Board-Certified Internist.
The Medical Specialist 2 will be supervised by the Taconic DDSOO Region 4 Medical Director and will have a base work location at 76 Firemens Way, Poughkeepsie N.Y.; additionally, the physician will be expected to rotate at least once per month, at the Boices Lane Article 16 Satellite Clinic site, Kingston N.Y., or occasionally at other certified satellite clinic sites, per the discretion of the Taconic DDSOO Medical Director.
The duties and responsibilities of the Medical Specialist 2 physician are outlined here accordingly and are delineated under the title of Article 16 Clinic Medical Director or State Operations Residential Physician as indicated below.
- The Article 16 Clinic Medical Director is responsible for the ongoing direction of clinical services provided by clinicians of the Article 16 Clinic. The Clinic Medical Director is licensed to practice medicine in New York State and is designated as responsible for maintaining the general health conditions and practices of the clinic
department program. Article16 Clinic Medical Director has been appointed at a sufficient full-time equivalent to provide adequate oversight of the constellation of services offered by the clinic facility for a clinic in operation five days or more per week. For clinics operating less than 5 days per week, coverage shall be at least proportional.
- The Article 16 Clinic Medical Director is responsible for approving all clinicians’ Clinical Treatment Plans within the agency’s Electronic Medical Record (EHR) sent by the Clinic Treatment Coordinator.
- The Article 16 Clinic Medical Director is responsible for reviewing and approving all requests for Clinic Treatment Services within the agency’s Electronic Medical Record (EHR) sent by the Clinic Treatment Coordinator.
- The Article 16 Clinic Medical Director is responsible for conducting, completing, and approving all Physician “MD” Annual Assessments within the agency’s Electronic Medical Record (EHR). MD Assessments must be completed and dated no later than 31 days after a full calendar year has elapsed since the date of the last completed physician reassessment. Not completing an Annual MD Assessment or completing the assessment late can result in a financial loss to the clinic from the time it was due until the time it was completed.
- The Article 16 Clinic Medical Director is required to participate in clinic case reviews and is responsible for clarifying information in patients’ medical histories; provide approvals for urgent consents when needed; write prescriptions for medications and adaptive equipment when recommended and clinically indicated; and when necessary, referring clinic patients to outside community physicians and specialists.
- The Article 16 Clinic Medical Director participates in policy development and implementation for the Article 16 Clinic, as appropriate and applicable.
- On Call after hours responsibilities as indicated by Taconic's Medical Director.
- Reviews patients for MOLST & OPWDD Checklist process Develops, implements, and reviews medical training intervention programs for staff, in consultation with nursing.
- Participates in weekly Human Rights Committee Meeting and other pertinent, as needed meetings, including but not limited to Mortality Review, Board of Visits, Surrogate Decision-Making Committee etc.
- Develops and administers procedures and policies for the medical specialty treatment program, facilities, and staff and assures that staff comply with them.
- Periodically, will back-up the Taconic Medical Director, in his/her absence.
- Reviews and familiarizes oneself with the legal rules and regulations pertaining to those with ID/DD, specifically the Health Care Decision Act, Surrogate Court Decision Act and NYCRR 633
- Obtains, reviews, updates, and records individual information, such as medical histories, test results, diagnoses, prescribed treatments, and individual responses; and clarifies information with individuals as needed.
- Conducts physical examinations; orders, performs, and interprets diagnostic tests and procedures; determines and/or confirms diagnoses; and treats a variety of specialized medical conditions.
- Prescribe and/or administer treatments, medications, therapies, vaccinations, and other specialized medical care to treat or prevent illness, disease, or injury; conduct follow-up examinations and monitor individuals’ conditions, modifying treatment as needed.
- May collaborate with other health care professionals and disciplines to achieve treatment goals and objectives and ensure continuity of care.
- Explains and discusses procedures, test results, diagnoses, and prescribed treatments with individuals, families, and caregivers, and answers questions.
- Refers individuals to other health care providers based on diagnostic and treatment needs.
- Participates in discharge planning and works collaboratively with other providers and disciplines as needed to facilitate medical follow-up after discharge.
- Maintains medical documentation consistent with OPWDD agency, New York State, and federal requirements.
- Supervises individual care activities; prescribes, orders, and oversees the administration of treatments as appropriate and depending on clinical situation.
- Consults with and advises other Medical Specialists, physicians, medical providers, and health care disciplines on diagnoses and treatment plans; and reviews and approves treatment recommendations.
- Provides clinical supervision, guidance, and training to other medical professionals, nursing, and direct care staff; and monitors the coordination and implementation of care.
- Reviews individual records to confirm diagnoses, and/or ensure that treatments are appropriate for diagnoses and that records are properly maintained.
- May assign individuals and individual care activities to other Medical Specialists, physicians, and/or medical providers, may approve leave, track time, attendance and complete performance evaluations, as delegated by Medical Director.
- Functions as a member of an interdisciplinary treatment team, attends meetings, and serves on various committees and workgroups to improve individual services and outcomes.
- Assists in the development, implementation, and revision of clinical policies and procedures.
- May develop, implement, and/or supervise residency training programs and specialized health care and treatment education programs.
- Will be expected to maintain a good time and attendance record, complete all mandatory trainings, and perform tasks as assigned by supervisor.

Minimum Qualifications Possession of a license to practice medicine in New York State (see limited license/permit option under notes) AND successful completion of a residency training program in your specialty approved by Accreditation Council for Graduate Medical Education (ACGME).
In order to be eligible for appointment and to maintain employment, you cannot be listed as an excluded individual or entity on any of the Federal and/or State Medicaid and Medicare exclusion lists (or excluded from any other Federal or Federally assisted program). If you are appointed and subsequently listed as an excluded individual or entity on any of these lists (or excluded from any other Federal or Federally assisted program), you may be terminated from your employment.
If appointed, you may be required to become an enrolled Medicare provider; obtain and provide to your employer a National Provider Identifier (NPI) number issued by the National Plan and Provider Enumeration System (NPPES); and otherwise actively participate to the degree necessary to allow for your services to be billed through Medicare and Medicaid. If you are appointed and you lose the ability to bill through Medicare and Medicaid, you may be terminated from your employment.
Loss of your license or limited permit or board certification in New York State or failure to maintain board certification may result in action to remove you from the position.
Limited license/permit option: If you are licensed in another state or by written examination in Canada, AND meet the other requirements for Medical Specialist 1 or 2, you are also eligible if you have a current limited permit to practice medicine in New York State. If appointed, you must obtain your license within one year of the date your limited permit was issued. Failure to do so will result in removal from your position.
Additional Comments This title is part of the New York Hiring for Emergency Limited Placement Statewide Program (NY HELPS). HELPS Program titles may be filled via a non-competitive appointment. This means that you do not need to take an exam to qualify, but you do need to meet the minimum qualifications of the title.
At a future date (within one year of permanent appointment), employees hired under NY HELPS are expected to have their permanent non-competitive employment status converted to permanent competitive status. You will not have to take an exam to gain permanent competitive status.
• If you are currently a New York State employee serving in a permanent competitive qualifying title and eligible for a transfer via Section 70.1 of the Civil Service Law, you may be appointed to this title as a permanent competitive employee.
• The assignment of PS&T employees will be made in accordance with any agreed upon Memorandum of Understanding between PEF and the local DDSO regarding the posting of job vacancies.
