Duties Description PRIMARY DUTIES AND RESPONSIBILITIES:

The Sunmount DDSO FCC is responsible for the day-to-day operational, fiscal and regulatory management of the Family Care Program spread throughout the six county district. Duties include significant travel, serving as an administrator on duty, conducting investigations, participation in OPWDD statewide Family Care Coordinator initiatives and supervising program staff. Specific responsibilities include:
Family Care
• Ensuring compliance with written policies and procedures relative to Family Care Home Certification.
• Ensuring that all providers are informed and trained regarding Family Care rules, regulations, policies and procedures including OPWDD staff, Health Home Care Managers, and FC providers.
• Ensuring that home visits are conducted in a timely manner by the Family Care Home Liaison and nurses.
• Provide necessary documentation to DQIPM to ensure the timely recertification of all existing Family Care homes.
• Serving as liaison, and/or monitoring, with the DDSO Quality Management regarding temporary suspension of operating certificates, home closure, application denial, and hearings.

• Ensuring that the Family Care Advisory Council meets on at least a quarterly basis, and that minutes are recorded, and distributed, in a timely manner,
• Ensuring that a current record is maintained on each Family Care provider.
• Entering or ensuring the entering of Family Care (dis)enrollments data into the Tracking and Billing System.
• Conducting and or ensuring the investigation of all incidents, and/or allegations of abuse, and neglect consistent with 14 NYCRR Part 624 Reportable Incidents, Serious Reportable Incidents and Abuse.
• Monitoring, and approving all payments made to providers, or investigating inquiries regarding late and missing checks.
• Working with the DDSO Business Office to ensure that payments for home modifications, provider, and other reimbursements are made in a timely manner.
• Monitoring, and approving respite services, trial visits, sitter services, overnight travel, out of state requests and therapeutic leave.
• Ensuring that the Residential Habilitation Plan is thorough and well written based on the individual’s needs.
• Planning annual recognition activities for Family Care providers at a place convenient for providers.
• Conducting Validation Surveys as requested by Central Office.
• Monitoring /overseeing and provide technical support to Agency Sponsored Family Care Programs.
• Directly supervise multi-disciplinary FC team.
• Review placement determinations to provide recommendations to upper level staff for the development of placement plans based on OPWDD census projections.
• Review established service levels for individuals with developmental disabilities, in order to verify appropriate levels of care.
• Analyze Statements of Deficiencies (SOD) issued to assigned agencies in order to identify trends in site non-compliance and to propose new policies and procedures to upper level staff to address deficiencies.
• Analyze performance reports for community-based programs and/or services in order to assist upper level staff in the development of new local programs and services to address unmet program and/or service needs.
• Serve as a point person to address programmatic or operational inquiries that arise in the delivery of programs or services for a variety of groups
• Will be included in the Administrator on Duty (AOD) rotation

• Will work closely with DDPSIV for Sunmount Community with:
o Investigations
o Site Visits
o Auditing Charts
o Training
o Program Development
o Committee participation
o Administrative coverage as needed
o Interviews for Clinical Candidates and Supervisors
o Liaison with voluntary Day Programs and CCOs
