Minimum Qualifications •Permanent, in title Community Mental Health Nurse
•Eligible for 70.1 transfer by way of current permanent status in a similar nursing title within 2 salary grades of a Grade 19
•The Candidate must be able to perform CPR compressions on a supine individual on the floor. The Nurse demonstration of CPR compressions on the floor would ensure nurse ability to perform in worst case scenario; since we use no backboards in OPWDD.
•The Candidate must be able to perform manual physical interventions of The Strategies in Crisis Intervention and Prevention course. The interventions include but are not limited to releases, holds and takedowns. Must be able to lift arms over the head, get on the floor in lying or kneeling position.

Provisional Qualifications:
Must be licensed and currently registered to practice nursing in New York State, or possess a limited permit to practice nursing in New York State. (You must obtain your New York State license within one year of the date of appointment or action will be taken to remove you from the position.)
AND EITHER
1.a bachelor's or higher degree in one of the areas listed below*; AND two years of professional clinical nursing experience, including at least one year as described below **; OR
2.30 college credits in the areas listed below* IN ADDITION TO an associate's degree in nursing or completion of a three-year nursing program (attach a copy of your college transcript(s) to your NYS-APP) AND three years of professional clinical nursing experience, including at least one year as described below **
* Acceptable areas include nursing, health care administration, public administration in a health care area, public health, community mental health, community mental health counseling, psychology, sociology, biology, social work or human services. The 30 credits may include up to two statistics courses. Courses/degrees in physical education, recreation therapy, English, history, math, anthropology, languages, etc. are NOT acceptable.
** One year of experience in providing direct services to individuals diagnosed with mental illness, mental retardation, developmental disabilities, or alcohol or substance abuse in a COMMUNITY SETTING, e.g., a halfway house, visiting nurse, or a community clinic. SUBSTITUTION: A master's degree in community mental health may be substituted for this experience only.

Duties Description Assignment Information:
Work as a residential nurse and manage 2 medically frail IRA’s and Supervise Senior LPN.
Duties to include but not limited to:
.Supervises overall individuals’ health care in assigned homes working with House Leaders, to implement optimal care. Assess individuals on an ongoing basis, identifying needs and updating discharge transition.
.Supervises the health care of individuals living in family care by working with the family care provider, to implement optimal care.
.Working with the residence House Leader to coordinate appointments for primary visits, consultant services and required tasks.
.Provides oversight to individuals when hospitalized including the discharge transition.
.Ensures that medications and treatments are administered according to approved policies and regulations. Provides ongoing oversight to staff administering medications.
.Provide nursing component to the Interdisciplinary Team.
.Provides training and oversight to staff in the area of medication administration, OSHA (Blood Borne and TB); and individual specific training needs.
.Conduct investigations as assigned by supervisor.

Additional Comments 8:00 AM- 4:30 PM Monday through Friday
-A total of 5 hours deviation from the default schedule
(hours before or after the regular work day).
-4 hours a month are expected to be on a weekend day and would count toward the above deviation expectation.ADDITIONAL QUALIFICATIONS:

• CAPA: Must be cleared by the NYS Central Register for Child Abuse and Maltreatment, if position requires working with minors and not previously cleared.
• All OPWDD employees must be eligible and maintain eligibility for full and unconditional participation in the Medicare and Medicaid programs. Continued employment will depend on maintaining eligibility.

The PSLF Program forgives the remaining balance on Direct Loans after 120 qualifying monthly payments have been made under a qualifying repayment plan while working full-time for a qualifying employer.
