At least one year of permanent service in a title eligible for 70.1 transfer pursuant to Civil Service Law

Reachable on the current New York State Civil service list for Social Work Assistant 3

FOR PROVISIONAL APPOINTMENT:

Must have a Master’s Degree in Social Work; or a Bachelor’s Degree in Social Work and one year of post-Bachelor’s experience in social work; or a Bachelor’s Degree or higher in a human service area and two years of post-Bachelor’s experience in social work

Duties Description The candidate selected for this position will act as the Self Direction Liaison. The incumbent will oversee the development of a
self-directed plan. Will work closely with the participant, family members, MSC, broker and fiscal intermediary agency. Will review
plans for accuracy. Will participate in local review committees and complete necessary paperwork for approval. Submit plans
electronically to Central Office. Will send approval letters and other correspondence to all parties. Will provide technical support
to brokers, MSCs and all involved in the development and maintenance of a self-directed plan. Will participate in “launch
meetings” when the plan is approved. Will ensure all parties are aware of their responsibilities and provide guidance on the
implementation of the plan. Will participate in ongoing statewide work groups and trainings for brokers and other interested
parties. Will work with other self-directed liaisons regionally to provide trainings for new brokers. Will track on-going training
requirements for brokers. Will provide person centered planning information as needed to interested service coordinators,
parents, brokers, etc. Will participate in forums and presentations to agencies and parent groups interested in self-direction.
