* Qualified Intellectual Disorder Professional (QIDP) designation requires one year's experience working with
people with a developmental disability AND a Bachelor's Degree or higher in a human services field (including,
but not limited to: sociology, special education, rehabilitation counseling or psychology).
MINIMUM QUALIFICATIONS:
Permanent Qualifications: Candidates must currently be a permanent Social Work Assistant 2 and QIDP OR reachable on the current Civil Service eligible list.
Transfer Qualifications: be eligible for transfer under Section 70.1 of the Civil Service Law.
Duties Description POSITION DESCRIPTION:
The Social Worker Assistant 2 (SWA2) provides oversight of the Family Care Program. The SWA 2 assists the interdisciplinary team members to ensure that individuals’ services are coordinated under the auspices of the residential habilitation and family care guidelines.
PRIMARY DUTIES AND RESPONSIBILITIES:
Duties include, but are not limited to:
o Covering a caseload of Family Care Homes
o Responsible for ensuring that the safety and health of the individuals are maintained in the Family Care Home (FCH)
o Monitors FCHs to ensure that any safety and health issues are immediately addressed and the required follow up is completed
o Conducts family care visits
o Monitors and develops residential habilitation plans and Health and Safety Plans
o Ensures communication between the Family Care Provider (FCP) and the other members of the interdisciplinary team
o Ensures that the FCPs are adhering to the family care policies
o Ensures that all policies and procedures are adhered to as per the family care manual
o Ensures that the FC policy 10.7.4 (Family Care Home Liaison Responsibilities) are adhered to
o Other duties as assigned
