Qualifying Experience for Appointment from the Eligible List

Either 1. a bachelor’s degree or higher in a human service area* and one year of permanent competitive 55-b/55-c service as a Social Work Assistant 2 or parenthetic;

Or 2. three years of permanent competitive 55-b/55-c service as a Social Work Assistant 2 or parenthetic.

Duties Description POSITION DESCRIPTION:

This position will be part of the DDRO Waiver Services Team, with an expertise in Employment Services

PRIMARY DUTIES AND RESPONSIBILITIES:

Program Coordinator
This position as a Program Coordinator is primarily responsible for processing requests for services in the eight county CNY district and ensuring the quality of services received. The primary emphasis will be on Employment (SEMP/Pre-Vocational/Pathway to Employment), but the position will also process service requests for Day Habilitation, Community Habilitation and Hourly Respite Services. The Program Coordinator must develop an expertise in the various Employment Services provided by OPWDD as well as through Acces-VR.
Processing requests for services includes: processing requests through CHOICES; communicating with Voluntary Providers to ensure availability of services; and monitoring units of service for the various programs.
This position serves as a liaison to the Voluntary Providers, ensuring quality of services and implementation of ADMs. The Program Coordinator assists agencies with questions and problems that may occur during service delivery.
Site visits may be required to assess service provision in the eight counties.
Working closely with Employment Training Program Coordinator, may serve as a member of various committees overseeing Employment services.
The Program Coordinator will be the CNY district’s workshop transformation liaison. The Coordinator will provide technical support to Voluntary Agencies to ensure the transformation is a smooth process.
This position will work collaboratively with Acces-VR and meet with them on an ongoing basis. Staff will also work closely with various agencies and organizations to help develop additional employment programs in the CNY district.
SELECTION CRITERIA

The successful candidate for this position will:

• Possess a working knowledge Care Coordination.
• Possess a working knowledge of HCBS waiver, individualized services, state-funded services, and generic community as well as OPWDD-sponsored human services and resources.
• Have experience working directly with individuals with intellectual disabilities.
• Demonstrate an ability to develop and present well-organized, and effective training/education to internal and external stakeholders in a cogent and professional manner.
• Demonstrate knowledge of CNYDDRO/OPWDD policies and procedures, especially those related to service development and standards compliance as it relates to OPWDD-funded services.
• Be flexible in their work schedule and able to adjust work hours to meet program needs & assist team members in response to urgent individual/family needs.
• Demonstrate empathy, respect, and warmth when interacting with those we serve, their families/advocates, colleagues within the Regional Office, as well as colleagues in Central Office, State Operations, and the other Provider Agencies the RO is associated with along with the public at large.
• Possess an ability to communicate in a thorough, concise, accurate, and professional manner both verbally and in writing, including the ability to speak in public to share information related to the RO and various Team functions.
• Have demonstrated an ability to manage time well and work independently, including having shown an ability to travel to and from various sites in a reasonable amount of time, ensuring that OPWDD and the Regional Office is consistently represented in a manner which exemplifies professionalism and honors the public trust.
• Possess a positive work history (E-file).
• Possess an acceptable attendance record as specified in CNYDDSO’s T&A Policy.
