QUALIFICATIONS
Permanent Transfer Candidates: Current DOH employee with permanent status as a Consultant Nurse, G-24 OR current NYS employee with one year of permanent or contingent-permanent service in a qualified title and eligible for transfer to Consultant Nurse Community Nursing and Home Health Services under Section 70.1 of the Civil Service Law OR eligible for transfer under Section 70.4 of the Civil Service Law (NYS employee with one year of permanent service at G-22 or higher and meeting the open competitive minimum qualifications listed below).
Provisional Open Competitive Minimum Qualifications: licensed and currently registered as a nurse in NYS; AND four years of public health, community health, or other nursing experience in an appropriate facility setting. Two years of this experience must have been providing direct patient care and two years of this experience must have been in nursing supervision, or working in the capacity of a nurse administrator.

QUALIFICATIONS
Professional experience in a community/home health setting with experience in supervision or administration of a federal or state health related certified program. Must have strong interpersonal skills, excellent decision making capability and strong writing and editing skills. Must have a strong knowledge of databases and be computer literate. Experience in home care is preferred, but not a must. Experience with interpreting regulations, conducting complaint investigations, conducting survey and monitoring health care programs’ compliance with rules and regulations.

Duties Description The Consultant Nurse Community and Home Health Services will report to the Director of the Division of Home and Community Based Services in Albany and serve as the Program Manager in the Western Regional Office (WRO) responsible for licensure, certification and surveillance activities of Home Care and Hospice programs in the 17 county region. The incumbent will provide administrative leadership for staff in the WRO by managing personnel; developing policies and procedures, information and communication systems; identifying staff training needs, developing and conducting continuing education activities for program staff; and other administrative functions. The incumbent will function as liaison with health care providers and the public. The incumbent will be responsible for reviewing surveillance activities conducted by regional office staff; tracking and monitoring home care complaints; ensuring consistency of the survey process; monitor program resolution with providers, regions and organizations; and interpret policy and statute in direction coordination with the DOH Central Office.

Additional Comments Permanent or provisional, full-time. Provisional appointees will need to take and pass the next Civil Service examination for this title and be immediately appointable from the resultant eligible list to maintain employment in position. This position involves travel throughout 17 counties which may include locations not served by public transportation.
