Minimum Qualifications Promotion: Candidates must have one year of permanent or contingent/permanent competitive or 55-b/55-c New York State service as a Social Work Assistant 3; OR reachable for appointment from the Civil Service List; OR eligible for a 70.1 transfer.

Open Competitive: Candidates must have a Master’s Degree in Social Work; OR a Bachelor’s Degree in Social Work and one year of post-Bachelors experience in social work; OR a Bachelor’s Degree or higher in a human service area and two years of post-Bachelors experience in social work.

Duties Description This position is for the Front Door/Eligibility and CCO transition unit for Metro Bronx DDRO. The incumbent’s job duties will include but are not limited to the following:

• As a Front Door Facilitator staff will complete Assessment and ensure eligibility criteria to enter the HCBS waiver has been met.
• Conduct Front Door information sessions
• Assist families with getting needed information to enroll them into waiver.
• Staff will conduct eligibility reviews and complete levels of care for individuals seeking waiver services.
