Minimum Qualifications MINIMUM QUALIFICATIONS FOR PERMANENT APPOINTMENT:

Candidates must be reachable on the current Civil Service List for Community Mental Health Nurse OR currently permanent in a title eligible for 70.1 transfer per Civil Service Rules & Regulations.

MINIMUM QUALIFICATIONS FOR TEMPORARY APPOINTMENT:

You must be licensed and currently registered to practice nursing in New York State or possess a limited permit to practice nursing in New York State. (You must obtain your New York State license within one year of the date of appointment, or action will be taken to remove you from the position).

AND EITHER

1. A bachelor’s or higher degree in one of the areas listed below*; AND two years of professional clinical nursing experience, including at least one year as described below**; OR
2. 30 college credits in the areas listed below* IN ADDITION TO an associate’s degree in nursing or completion of a three-year nursing program AND three years of professional clinical nursing experience including at least one year as described below **

*Acceptable areas include nursing, health care administration, public administration in a health care area, public health, community mental health, community mental health counseling, psychology, sociology, biology, social work, or human services. The 30 credits may include up to two statistics courses. Courses/degrees in physical education, recreation therapy, English, history, math, anthropology, languages, etc., are NOT acceptable.

**One year of experience in providing direct services to individuals diagnosed with mental illness, mental retardation, developmental disabilities, or alcohol or substance abuse in a COMMUNITY SETTING, e.g., a halfway house, visiting nurse, or community clinic. SUBSTITUTION: A master’s degree in community mental health may be substituted for this experience only.


Duties Description POSITION DESCRIPTION: Care At Home Coordinator


PRIMARY DUTIES AND RESPONSIBILITIES:
Job responsibilities include but are not limited to:

• Coordination of Care at Home Program for children diagnosed with developmental disabilities and complex health care needs;
• Phone referral and completion of the level of care screen form;
• Assist family to acquire OPWDD eligibility;
• Organize enrollment process: medical information, required documents and complete a home visit;
• Interface with Nassau and Suffolk County DSS offices and school district staff;
• Complete Annual Level of Care Assessment to verify continued eligibility, which requires a face-to-face visit with family and child;
• Review Semi – Annual Home Assessment Abstract for Quality assurance. Ensure compliance with requested corrections;
• Provide direction with regard to Environmental Modification and Assistive Technology proposals;
• Oversee appropriate case management services;
• Coordinate Termination of Services, which includes sending a Notice of Decision to family, the appropriate DSS office, and to the Statewide Care At Home Coordinator;
• Attend and assist in preparation of expert testimony at Fair Hearings;
• Provide continuing education to Case Managers and Supervisors on a regular basis to disseminate information and review regulations for continued compliance;
• Attend NYS Care at Home meetings, Health Fairs, Schools & Hospitals to provide outreach to communities, organizations and families;
• Provide technical assistance to the Front Door as well as In-Service training for families and professionals;
• Assist in HCBS Waiver Transition;
• Preparation of documents for audit and waiver renewal

Additional Comments Posted Hiring Salary includes the sum of the statutory hiring rate for PS&T Grade 19 ($58,501), an Increased Hiring Salary ($3,651), Geographic Pay Differential ($12,871), and Downstate Adjustment ($3,026). Geographic Pay Differential and Downstate Adjustment are added to the job rate ($74,301) for PS&T SG-19.

Will be required to flex schedule to accommodate late afternoon visits.
