FOR PROVISIONAL APPOINTMENT:

Must have a Master’s Degree in Social Work; or a Bachelor’s Degree in Social Work and one year of post-Bachelor’s experience in social work; or a Bachelor’s Degree or higher in a human service area and two years of post-Bachelor’s experience in social work

Duties Description The candidate selected for this position will act as the Front Door Facilitator (FDF). A FDF has the responsibility for providing a
welcoming environment and a positive experience for individuals/families/advocates requesting services. The incumbent will be
working with families who are interested in obtaining OPWDD services. Will complete the Eligibility Assessment Authorization
tool, Developmental Disability Profile 2 based on conversation with family/individual. Will provide MSC Agencies with
documentation to develop a plan for appropriate services for the individuals. Will write letters to agencies and families as part of
the Front Door process. Will track caseload using Front Door spreadsheet. Will respond to telephone inquiries regarding Front
Door/Eligibility Process. Will participate in Front Door Family and Individual Information Sessions. Will participate in the Front
Door Quality Review Team Process. Will assist the individual/family/advocate to objectively consider an array of possible service
options both OPWDD and non-OPWDD funded providers so informed choices can be made. Must have knowledge of the
OPWDD Eligibility Process. Must be familiar with Tracking and Billing System.
