Duties Description THE POSITION:
The Office for People With Developmental Disabilities (OPWDD) is seeking a START Coordinator for the NY START program in OPWDD DDRO Region 3. The START program addresses the need for available community based crisis prevention and intervention services to individuals with intellectual/developmental disabilities (I/DD) and co-occurring behavioral/mental health needs.

The NY START Region 3 team provides person-centered clinical services and therapeutic emergency and planned services. Through regionally-based START Clinical Teams, START Coordinators provide 24 hour a day, 7 days a week telephonic and/or in-person responses to the system of care in support of individuals with IDD and behavioral health care needs as well as promote the development of least-restrictive, life enhancing service supports to people referred.

The program is supported by multi-level linkage agreements between agencies and providers; ongoing clinical education and consultation; technical assistance; and, data-driven, evidence-informed practices and analyses. The NY START model requires adherence to a strict level of fidelity to the national START model and its requirements
and protocols for START Coordinator Certification, training, clinical excellence, crisis response, data collection and analysis.

The ideal candidate for this position will share OPWDD’s commitment to advocating on behalf of people with developmental disabilities as well as advancing and enhancing services and supports. PRIMARY DUTIES AND RESPONSIBILITIES:
The START Coordinator is the first point of contact for clients and the community. Coordinators link service providers with individuals in need, and work to improve the functioning of existing service elements in the community. Responsibilities will include, but not be limited to the following:

• Maintain linkages and relationships with community partners to provide consultation to systems/teams as needed using best practices.
• Must successfully participate in and complete the START coordinator certification training within 12 months of employment. This includes the demonstration of competencies in all areas associated with START coordination
• Ensure the coordination of support meetings and crisis plan development for individuals enrolled in NY START.
• Share on-call responsibilities on a rotational basis to ensure 24/7 telephonic and in-person response to crisis situations within two hours of call.
• Participate in recurring meetings with START leadership, clinical team, and Resource Center program.
• Develop Comprehensive Service Evaluations (CSEs), Cross-Systems Crisis Prevention and Intervention Plans (CSCPIPs), intake/assessments, admission/discharge summaries for the Therapeutic Resource Center and any other applicable documentation of services provided.
• Maintain an active caseload of approximately 10-15 cases (Low to average acuity cases).
• Understand the programmatic and cultural nuances found across different systems of care.
• Possess the vision to conceptualize the needs of individuals with I/DD outside of traditional institutional models of care and work holistically with individuals and systems.
• Ensure all documentation and SIRS data entry is complete and contemporaneous.
• Develop materials and present cases at Clinical Education Team meetings as assigned by the director or designee.

Travel: Travel throughout the Capital District catchment area will be required and at times may require travel to Hudson Valley and/or Taconic regions.

On-Call: Required - Shared on-call responsibilities to cover 24 hours per day, 7 days per week, both via telephone and in-person.

PREFERRED QUALIFICATIONS (not mandatory):

• Experience working with individuals who have challenging behaviors and psychiatric disorders in addition to Intellectual and Developmental Disabilities.
• Excellent oral and written communication and networking skills.
• Understanding of behavior support plans and data collection.
• Proficient computer and technology skills

Additional Comments SELECTION CRITERIA:
• Upon successful completion of the probationary period in the Licensed Master Social Worker 1 position and achievement of national certification, the incumbent may be eligible and promoted to the Licensed Master Social Worker 2 title.
