Minimum Qualifications MINIMUM QUALIFICATIONS:
• Must be currently permanent in the title of Social Worker Assistant 3. OR
• Must be active and reachable on the appropriate Civil Service eligible list. OR
• Must be eligible for 70.1 transfer by way of current permanent status in a title deemed comparable by the NYS Department of Civil Service.
• Must possess
A Master’s Degree in Social Work from a regionally accredited college or university.
OR
A Bachelor’s Degree in Social Work and one year of post bachelor’s experience in Social Work.
OR
A Bachelor’ s Degree or higher in a human service area and two years of post-bachelor’s experience in Social Work.
Duties Description PRIMARY DUTIES AND RESPONSIBILITIES:
1. Process approvals and terminations for all waiver services - community habilitation, respite, day habilitation, respite, day habilitation, supported employment, etc.
2. Review supporting documents related to the above transactions.
3. Generate Notice of Termination forms when applicable and distribute to appropriate parties.
4. Provide technical support to agencies and follow up with agencies regarding submission of documents.
5. Participate in required trainings and in-service trainings regarding the enrollment process.
6. Additional responsibilities to be determined based on unit needs.
