Provisional Appointment: In the event that there is not a valid eligible list, candidates who meet the qualifications below may be qualified for a provisional appointment.

Minimum Qualifications: Either A. licensed and currently registered by New York State as a Licensed Master Social Worker (LMSW) AND have one year of post NYS licensure professional social work experience;**
Or B. licensed and currently registered by New York State as a Licensed Clinical Social Worker (LCSW).

Duties Description The Licensed Master Social Worker 2 will fulfill the Residential School Coordinator Liaison/Family Support Services Liaison functions and will work within the Regional Office team to:
• Assist Individuals aging out of residential schools and their families in accessing OPWDD supports and services;
• Provide technical assistance to agencies to ensure continued compliance with waiver service requirements required for transitioning youth to adult services, review preliminary adult service plans and provide feedback to provider agencies to ensure optimal transition of youth into adult services;
• Act as the point of contact, regarding adult services for students aging out of Residential Schools from across New York State, as well as out of state schools;
• Track placement progress using CITIX data-base and any additional databases as required;
• Provide over-the-phone Front Door orientations to OPWDD services;
• Provide technical assistance to Medicaid Service Coordinators on the planning process;
• Communicate current OPWDD processes to Individuals/Families and the public at large;
• Travel to Residential Schools to meet students and assess their needs;
• Coordinate Service Planning Meetings with Families, Residential Schools, and Voluntary Providers;
• Participate as a member of the Certified Residential Opportunities Committee;
• Participate in Statewide Age Out Liaison meetings/calls and share information with the Regional Office Management team;
• Prepared detailed reports for Regional Office Management and Central Office Administration
• Review Letter of Interest (LOI) applications related to certified residential development for youth aging out of Residential Schools;
• Serve as Family Support Services Liaison, scheduling/facilitating Family Support Council Meetings;
• Participating in site reviews of Agency sponsored FSS contracted programs to ensure quality services;
• Other duties to be assigned based operational necessity

• CAPA: Must be cleared by the NYS Central Register for Child Abuse and Maltreatment, if position requires working with minors and not previously cleared.
• All OPWDD employees must be eligible and maintain eligibility for full and unconditional participation in the Medicare and Medicaid programs. Continued employment will depend on maintaining eligibility.

If you are employed by state government you may be able to receive loan forgiveness under the Public Service Loan Forgiveness Program (PSLF). The PSLF Program forgives the remaining balance on Direct Loans after 120 qualifying monthly payments have been made under a qualifying repayment plan while working full-time for a qualifying employer.
