For Provisional Appointment (you MUST have taken the NYS Civil Service Social Work Assistant 3 exam held on May 13, 2017 to be considered for provisional appointment):
1. Candidates must have one year of permanent competitive or 55-b/55-c service as a Social Work Assistant 2 or parenthetic AND a Bachelor’s Degree or higher in social work; OR, two years of permanent competitive or 55-b/55-c service as a Social Work Assistant 2 or parenthetic AND a Bachelor’s Degree or higher in a human service area; OR, three years of permanent competitive or 55-b/55-c service as a Social Work Assistant 2 or parenthetic.
OR
2. A Master’s Degree in Social Work; OR, a Bachelor’s Degree in Social Work AND one year of post-Bachelor’s experience in Social Work; OR, a Bachelor’s Degree or higher in a human service area AND two years of post-Bachelor’s experience in social work.
Duties Description POSITION DESCRIPTION:
The successful candidate for this position will serve as an integral member of the WNY Regional Office Front Door Team. The
candidate will assume responsibilities that will include, but not be limited to, promotion of the OPWDD Transformation Agenda
and Goals; specifically to provide information and training to individuals, families, and provider agencies regarding self-direction,
employment, day services and residential options, in addition to other OPWDD initiatives. The candidate will be required to work
flexible hours when needed to accommodate individuals and families.
PRIMARY DUTIES AND RESPONSIBILITIES:
o Serve as a Front Door Facilitator and process requests for access to OPWDD services
o Complete an assessment of needs and current supports
o Provide information to individuals and families regarding OPWDD services and additional community supports
o Participate in the Quality Review Team process
o Maintain electronic record and tracking of Front Door contacts (Eligibility, Assessment, and Authorization Tool)
o Complete preliminary individualized service plan to authorize services and supports
o Assist with providing Front Door Information Session trainings for individuals, families and provider agencies
o Additional duties and responsibilities as needed and related to OPWDD/Regional Office functions
