Minimum Qualifications You must take and pass a Civil Service examination in order to be considered for this position.
Candidates must have:

• Possession of a license to practice medicine in New York State; AND
• Possession of a M.D., M.B.B.S. or D.O. degree; AND
• Certification by the appropriate American Specialty Board; AND
• Eligibility for full and unconditional participation in the Medicaid and Medicare programs.

Duties Description Bronx serves 194 individuals in our State Operated homes and 102 individuals in our three Day Habilitation Programs all which requires physician over sight and medical coordination. The Medical Specialist 2 will be responsible for two medically enhanced residences serving a total of 48 individuals with diagnosis of profound developmental disabilities, scoliosis, medically frail, vision and hearing impaired and challenging maladaptive behaviors. He/she will also have 9 smaller residences to cover (57 individuals). Total caseload is 105 individuals to provide medical coordination oversight for.

Prospect Avenue Home (three apartments with 8 individuals each [24] has individuals who are visually impaired, hearing impaired and have maladaptive behaviors. There are frequent hospitalizations, URIs and UTIs. Some individuals have challenging behaviors which require monitoring of psychotropic medications and balancing them with other prescribed daily medications to assure safe interactions and levels. Majority of the people requires total assistance with all of their ADL needs.

Union Avenue Home (three apartments with 8 individuals each [24] is very similar to our Prospect Home with the same/similar care needs for the individuals. Majority of individuals require medical oversight of an in house physician for more than a few hours a week to become familiar with each individual so that any signs of physical decomposition can be more quickly addressed; to ensure that medical principles and practices are followed and that care plans are made to suit the individual’s needs; to constantly be in communication with the nurses and staff so that care can be optimized.

The remaining five homes range from a certified capacity of 1 to 14 individuals. The individuals have varying levels of intellectual ability, independence and supervisory needs with the majority requiring 24 hours supervision in the home and community.

There is a need for a great deal of administrative work that must be completed, reviewed and “signed” off by the physician which can be expedited with the doctor being on sight in the home. Medical Specialist 2 will be able to effectively communicate with the primary clinic and hospital physicians on a collegial level (meeting in hospital when necessary) and advocate for the individuals, clarify the care plans while the individual is hospitalized and before discharge. The physician is a vital participant at the table for end of life discussions as well as be available for meetings with families. Quality of care and standards will suffer if there is on required physician on duty in these homes.
