Minimum Qualifications A bachelor’s or higher-level degree in a human services field*

* a degree in a human services field includes the professional disciplines of audiology, community mental health, nursing, nutrition, occupational therapy, physical therapy, psychology, rehabilitation counseling, social work, speech/language pathology, and therapeutic recreation (including concentrations in art, creative arts, dance, drama, music, or physical education.) Also included are related academic disciplines associated with the study of human behavior (e.g., gerontology, human services, sociology, speech communication); or human skills development (e.g., counseling, education, human development.) Examples of non-qualifying degrees include but are not limited to: criminal justice, political science, liberal arts, and business.

Duties Description • Works as a member of the interdisciplinary residential team.
• Schedule and attend annual and semi-annual Life Plan meetings as needed.
• Assist with development, revision, and implementation of Residential Habilitation/Staff Action Plans.
• Ensure completion of monthly progress notes and reviews billing documentation to ensure accuracy.
• Develop/review/revise Safeguard Summary Plans as needed.
• Complete PEPs at the Annual Life Plan meetings and as needed.
• Coordinate special meetings with outside agencies/advocates to review the individual’s plan.
• Review monthly checklist, data sheets, discrepancy report and follow up on billing discrepancies.
• Conduct on-site reviews of documentation to monitor billing compliance.
• Maintain Residential Habilitation records in assigned IRAs. Caseload may change based on operational need.
• Provide ongoing training to IRA staff regarding Res Hab implementation, documentation, safeguard summary.
• Prepare Residential Habilitation documentation for audits.
• Establish working relationships with Care Managers, clinicians, day/work programs to ensure Life Plan meetings are conducted and services are coordinated.
• Additional responsibilities as assigned by supervisor.
• Travel required.
